The SHBG (Sex Hormone Binding Globulin) test measures the level of sex hormone binding globulin in the blood. SHBG is a protein produced primarily by the liver that binds to sex hormones — mainly testosterone, dihydrotestosterone (DHT), and estradiol — and transports them through the bloodstream. Only hormones not bound to SHBG (free hormones) are biologically active and available to the body’s tissues.
This test is used to evaluate the amount of biologically available testosterone or estrogen in the body. It is commonly ordered alongside total testosterone to calculate free testosterone levels. It helps diagnose conditions such as polycystic ovary syndrome (PCOS) in women, hypogonadism in men, infertility, irregular menstrual cycles, and symptoms of androgen excess or deficiency. SHBG levels are also affected by obesity, thyroid disease, diabetes, and liver conditions.
