What is Total Bilirubin test?

The Total Bilirubin test measures the combined level of both direct (conjugated) and indirect (unconjugated) bilirubin in the blood. Bilirubin is a yellow pigment produced during the normal breakdown of red blood cells in the spleen and liver. It is processed by the liver and excreted into bile and ultimately into stool.

This test is commonly used to evaluate liver function and to diagnose conditions causing jaundice — a yellowing of the skin and eyes. Elevated total bilirubin may indicate liver disease (hepatitis, cirrhosis), bile duct obstruction (gallstones, tumors), or hemolytic anemia (excess breakdown of red blood cells). It is often ordered as part of a Liver Function Test (LFT) panel alongside direct bilirubin, ALT, AST, ALP, and albumin. In newborns, bilirubin testing is critical to detect and manage neonatal jaundice.
